If you will be using insurance to pay for your visit, you must remember to bring your insurance card as well as a valid photo I.D. to the appointment. If your plan requires you to pay a co-payment then this will be due when you check in for your appointment. This can be paid with cash, a check, Visa, MasterCard, American Express, Discover or Care Credit. If you are covered by a plan not listed below, we would be happy to give you a statement to submit to your insurance company so you can be reimbursed for your visit. Nevertheless, in such cases, the full cost of the visit will be due at the conclusion of your appointment.

What is a deductable?

A deductible is an agreement between you and your insurance company in your contract that must be paid from the patient before the insurance pays the provider.

What is a coinsurance?

Coinsurance is an agreement between you and your insurance company in your contract where the insurance company pays a percentage of the physicians charges and the patient is responsible for the other percent.

What is an out of pocket maximum?

Out of pocket maximum is an agreement between you and your insurance company in your contract where after the patient pay the agreed amount of money within the contract year, the insurance company will pay 100% of the provider bills.

Do you have information out my out of pocket maximum, or where can I find this information?

Since everyone’s contract is different, the provider does not know about your out of pocket maximum. You can contact your insurance company (the phone number on the back of the insurance card) to obtain this information.
